The aim of the rapid response service is to prevent unnecessary hospital admissions and ease pressures on health services during the year by enabling individuals to be cared for at home instead of being admitted to hospital. There are specially trained care workers who will respond to the individual within an hour of call out to provide out of hours’ emergency care at home.
Referrals are placed by A&E departments, out of hours GPs, district nursing services, NHS Direct and walk in centres and the Social Services Emergency Duty Team. A member of our assessment team would be happy to visit you to discuss your needs.
