A key objective of the service is to enable individuals to manage their own service and how it will be delivered by making their own choices. Other specific objectives include promoting independence and encouraging people to have an active presence in the community. 

The teamwork with the Service User to provide weekly care plans to achieve the agreed outcomes, which might include personal care, meal preparation, and shopping, social inclusion and community integration. A member of our assessment team would be happy to visit you to discuss your needs.
